Alerting signals, symptoms, evidence of occurrence: 
Anti-group B IgG and IgM antibodies were found in patient's serum. On day 16, the helmet cells appeared in the serum and the patient required transfusion. Biopsy of the transplanted kidney was performed, which revealed thrombotic microangiopathy.
Demonstration of imputability or root cause: 
The patient whose blood group was B-positive, received a kidney from a group O-positive donor, resulting in anti-B antibodies, helmet cells and thrombotic microangiopathy, successfully treated through transfusion and plasma exchange.
