Acute febrile and systemic reaction to the transfusion.
Demonstration of imputability or root cause: 
Crossmatch using post-transfusion sample was negative at immediate spin but 3+ incompatible at AHG phase. Post-transfusion sample positive for IgG DAT and demonstrated agglutination with Kpa positive cells. Patient types as Kpa negative and donor cells were Kpa positive.
