Estimated frequency:
Most recent risk assessment for uterine and cervical cancer (Council of Europe, 2022): The presence of invasive uterus or cervix cancers is considered an unacceptable risk for organ donation. Uterus or uterine cervix cancer in the donor history: After a disease-free interval > 5 years, the transmission risk of invasive uterus and cervix cancers will depend on the probability of cure and has to be assessed individually before accepting the potential donor; no specific recommendations are available from the literature.
Time to detection:

Alerting signals, symptoms, evidence of occurrence:
Followup of previously reported transmission to liver recipient (Detry et al. Transplant Int 1993; 6:50). Donor autopsy performed the day after transplant showed metastatic squamous cell carcinoma originating from the uterine cervix. Recipient asymptomatic but allograft liver removed and patient re-transplanted on day 7; no tumor found in allograft hepatectomy. No evidence of cancer in recipient at 7 year followup.
Demonstration of imputability or root cause:
No evidence of transmission. Metastatic cancer found on donor autopsy.
