Alerting signals, symptoms, evidence of occurrence: 
Transmission of HCV. Co-transmission with HIV to multiple recipients by multiple organs. Three months after transplant: elevated liver enzymes in one kidney recipient; 10 months after transplant routine testing revealed HCV RNA positivity in the other.
Demonstration of imputability or root cause: 
Genetic sequences were highly related between between HCV and HIV strains infecting all 4 recipients, confirming transmission from a common donor. HCV in sera of all recipients was genotype 1a. HIV gag and evn sequence from donor and recipients were identical except for a single nucleotide difference found in liver recipient. Donor was negative for HCV and HIV by serology; had engaged in MSM behavior.
