Alerting signals, symptoms, evidence of occurrence: 
Wound infection, arterial anastomosis dehiscence, and death. Nephrectomy was decided on day 52 due to graft failure. Renal tissue grew Bacteroides. External iliac artery bleeding due to Bacteroides infection 11 days after nephrectomy.
Demonstration of imputability or root cause: 
Definite. Contaminated kidney caused recipient infection and graft loss. Preservation solution culture grew “lactose fermenting coliforms and bacteroides”. The same coliforms appeared in recipient urine and Bacteroides in wound culture. Additionally, arterial patch on postmortem exam, showed "gram negative organisms" and bacteroides.
