Alerting signals, symptoms, evidence of occurrence: 
Donor known positive, recipient under surveillance. Positive PCR at week 8 with no signs or symptoms. Additional cited case series with donor positive - recipient negative liver transplantation with active surveillance of recipients after transplantation. 3 of 9 recipients seroconverted and were treated.
Demonstration of imputability or root cause: 
Donor tested positive by IFA 1:256, multiple recipients positive during surveillance testing and both were treated once T. cruzi PCR was positive. Seroconversion in case series of 9 recipients demonstrated in 3 of the 9, prompting treatment.
